Discharge Planning

Upon your admission to ACHIEVE our team of professionals has already begun to plan your return home. Our Case Manager and Social Worker, along with the entire inter-disciplinary team, provide the necessary evaluation to determine your needs as an outpatient upon your return home.

Patient Education

Our nutrition, nursing and therapy departments provide the necessary patient education to assure family members of a safe discharge and return to an independent lifestyle.
