About Us

Welcome to Achieve Rehabilitation. At Achieve Rehab and Nursing Facility, we provide outcome oriented treatment techniques, for both long and short-term residents. Our skilled interdisciplinary team, consisting of doctors, nurses, physical therapists, speech therapists, dieticians, social workers and recreation personnel, constantly reviews our residents’ “plan of care” and monitors changing needs on an individual basis. Our Social Services Department assists with the transition to life at Achieve, for both long and short-term residents. When residents are ready for discharge, our Social Services Department works with various agencies to secure services providing for a pleasant transition back into the community.

Our facility uses a friendly approach to healthcare. Everyone on the staff works to create an environment where people can be people, even though their normal lifestyle has been interrupted by illness or injury.
